I know a number of other people were having the same issue as me on this - even the smallest OC leads to BSODs in 3DMark05 (and EQ2 in my case) that seem to be related to USB ports.

Did any of them solve this?

One thought I had was that it may have been due to using USB Keyboard and mouse - is anyone else using these and having no OC problems?

I have so many USB devices plugged into my A8N-SLI that I have had to add another USB PCI card cos I ran out of ports. I run my 3500+ at 234x11 = 2572MHz with no problems. What WAS giving me problems was that cheap chipset fan on the nforce4 - I whipped that off and replaced it with a Vantec Iceberq and my overclocks and stability improved.

Holy crap! You've used up the 10 ports on the board?!

I use a USB mouse and keyboard without any problems. I connect up to four other USB devices as well, but only two of those are always connected.
