You used to be able to do it by email, but that was discontinued a couple months ago (at least.) However, you can do it "electronically" by going to the T-Mobile "customer help" link and filling out a request. Go to http://voicestream.custhelp.com/cgi-bin/voicestream.cfg/php/enduser/ask.php and fill out the form. Initially you'll get an auto bot response telling you that you'll receive a response to your question/request. It's likely that within the same mail poll you'll get an answer to your request for unlocking. They will tell you that you need to be a customer for at least 90 days, have an account balance of less than $100, the name must match the billed party name. They will then ask you to provide the IMEI (*#06#), your mobile number and account security (last four digits of your SSN.) I'd also include the manufacturer and model number of the phone you're wanting to unlock as well. If you make your request in the morning you may possibly have the code sent to you by the afternoon. HTH.
