Got this in an e-mail today with the following info:

This photo was taken in a hospital after the patient was in an accident where he was responsible for a young woman's death. It is said that when you receive this image and do not send it to at least five people, the woman will look for you during the night to collect your soul. People in Laredo, Texas, received this image and did not send it and were killed outside a bar; it looked as if this woman killed them. Send it to five people or the woman will look for you.

It's a pretty obvious chop, the lighting doesn't seem to be quite right. Looks like something from the ring or exorcist or scary movie 2 chopped with some nice, sweet hospital website picture.
 
Trust me, I don't forward chain letters. The main reason I'd like to know what movie it's from is to send it back to the person who sent it to me. I find that replying to this crap with a link to the page at Snopes keeps my chain-letter load at a minimum.
