Anyone have the Monarch Rebate form for evga 7800GT?

Hi All,
I guess I did a stupid when I ordered the evga 7800GT from Monarch on Thanksgiving day and did not print out the rebate form at the same time. I guess I am used to being able to print out the rebate form after I get the item and I have the UPC code, receipt, and everything else in front of me. Problem is, Monarch no longer has the rebate form. I bought the N515-AX model. I would sure appreciate it if you could e-mail me a copy of the rebate form. I won't stop feeling stupid, however. Hmmm... I printed mine out the same day (I ordered a couple days before thanksgiving). I dont know if I actually downloaded it though. I will look for you when I get to me GF's house tonight.

Thanks, Matt. I finally found it! Thank goodness for Google's cached pages -- that's the only place it was available. I've got it filled out and will be sending it in today. $20 isn't a big deal, but hey, I can get about 1/2 tank of gas for that... Thanks again.

Yeah, I did the same thing. Had to google my rebate form after the fact.
