How is it predictable? How many other doctors on scifi shows smoke? Dr. Tam doesn't smoke in Firefly, Dr. McCoy didn't Dr. Crusher didn't, Dr. Bashir didn't, The Doctor didn't.

Even in real life a lot of doctors smoke. I even know a cardiologist that enjoys a cancer stick between cases.
