It's surprising how terrible SharePoint really can be considering what you pay for it in licensing costs. I'm not sure how much of that is due to incompetent admins or the software itself though.
You forgot a very high probability third option: senior managers who don't know the difference between a Content Management System and a Document Management System. In practice Sharepoint generally gets used as a giant trash can that everything is thrown into with no control processes like version control or creation of metadata for documents, then people complain about how they can't find shit on the Sharepoint site.
