Anyone ever fileld out a WebAVA survey for a business you are applying to?

I am in the process of doing it and am curious what it is used for.

One page asks me to use words that have described me.

There are positive, neutral, negative, and then words like : fashionable.

Should i say every word I have ever been called?

Do I leave out things like fashionable?

What do you think?
