Just had a stick fall on the hood of my car and put a nice 1.5" diameter dent in it, the roof also has a few scratches.

Went down to the insurance agent and was told to go get a quote. Quote came to $700. My comprehensive deductible is at $250.

Who does the insurance agent make the check out to? The company I got the quote from or me? The dent is very noticable, but the car is 6 years old. Nothing that needs fixing and I could think of better ways to spend the $450.

Didn't ask the agent because I felt guilty

They'll make it out to you, if you don't have a loan on the car. If you do, they'll make it payable to you and to the lienholder.

The bank's. They have to be concerned about resale value, just in case they have to repo it. You can put the money from the insurance towards to principal owed, if you don't want to have it fixed.

My insurance company (State Farm) would send you to one of their approved body shops.

You would not get a check.
