I've done it before, didn't seem all that hard.

People all over the world drive manual cars in hilly cities
And they seem to manage

I think just driving around is somewhat managable.
The parellel parking on steep hills when you have to squeeze between two tight cars though. . .that earns you your man card right there.

This is tough even in some automatics. I cringe everytime I hit the gas and the car doesn't actually move so I have to push more.

I have friends who have lived there and done it. It's not too bad if you don't really need to go to certain areas in the city. Most driving in SF isn't really that bad.
My last drive up there to the Nob Hill Masonic Center going up Taylor St there was a really nasty incline I had to stop at on a red light. There was an RSX stopped ahead of me loaded with passengers and he just couldn't get going. 8 or 9 burnouts and one missed traffic cycle later he finally made it heh.
It was so steep that the hood scoop covered the entire view of his car. I'd certainly hate for any inclines like that to be part of my daily commute. Most people I know that live and have work/school in the city just take public transportation anyway though.
