Anybody feel up for critiquing my registration form?

I've been working on a jQuery Dialog and ajax driven registration form. I don't want to blast the url but if anybody feels like hacking at it and offering criticism, I'd greatly appreciate it. PM for the url if you are so inclined. I'll delete all login records at the end of the day so it won't be any kind of permanent registration.
