Any Word Templates for Declining Promotion?

If you needed to "respectfully decline a promotion", how would you draw up the document to be filed? Just a couple sentences, dated & signed, or what? Anybody feeling eloquent?

i just told my boss, no, i don't want a "promotion"

reason i didn't take it, 1) parking sucks downtown, 2) i have to wear a suit.
glad i didn't take it as 2 years later, that position got out-sourced.

I need something they can file in their records. Guess it could be super simple, but it would be a hoot if there was a nice template to work from.

Thank them for the consideration, explain the reason you declined, end with a positive note on what you plan to do in you current position.

Just write it like a business letter. Date right-aligned in the upper corner, company address, full justification on the body, etc.

Dear Mr.,

After much consideration I must respectively decline your offer for promotion to (inset position here). I have come to the conclusion that at my current position I can be a greater asset to (inset company/organization etc) The promotion that you have so graciously offered me does not allow me to effectively continue to work with the accomplishment, passion, and determination that I currently bring to (organization) everyday.
