Any way to create a survey/form that also does math?

So I'm working on a survey for people to fill out to order some items we're doing as a group. The survey is for everyone to enter in what they want to order. I can do this easy enough with Google docs or surveymonkey or whatever.

However, I also need a way the form can do math based on what people choose.

For example, in one section you can choose from items that are 3 pounds or items that are 1 pound, as long as it adds up to the right total. So I want people to enter a number into the boxes and have it show the total on the form.

I know I could rig up a form in html but it wouldn't look very good so I'm looking for any sort of online system that can do this.

I could also do this in a spreadsheet very easily and have calculations that run as people enter the data, but I wouldn't have a way to make sure people aren't accidentally messing with the other rows in the form. I'd rather not have to make 50 copies of the same spreadsheet just for that. If there was a way to have a spreadsheet that only shows you your own row or that would generate separate instances for each person to click the link, that'd work too.

Does this make sense? Know of anything that can do this easily?

Those will make a form but i can't have the form do math on the form itself can I?
