If you have MS Office 2003, you can "print" it to mdi. Which is microsoft's version of PDF. Then you can add textboxes and stuff to the MDI and type in it.
Alternatively, you could use a program like PDF Creator (http://www.mrbass.org/freeware/pdfcreator/). Similar to what Codewiz mentioned, you can print to PDF from ANY application.
It's definately not the easiest way, but you could scan the original PDF and fill in the blanks using a graphics program of your choice, and then print it back out as a PDF.
