Many of the train issues are the equipment; Maintenance of the cars and track costs $$$. Physically issues are not easy to detect and very expensive to find let alone repair.

Cost benefit analysis is done - just like the auto companies and by every other bean counter.

This is actually a very huge problem and the train companies themselves do not want to do anything. The benefits are huge but a moderate cost is hated by the train companies. There was a very in depth article I read on the subject a few years ago.
