It is very likely safety does normally take a back seat in these plants. So it could very be normal, a sort of shitty normal when certain incidents can be risky injury-wise to workers or first responders. The first link was from another source. I don't really follow any specific source. I just linked that one too because it has the full list. Feel free to google each one if you really think they are just being made up. A lot of them are also on more than one source.
