an unauthorized charge on my credit card

Just found an unauthorized charge on my credit card made three weeks ago from Victoria Secret of OH. I never shopped with them and obviously the number is stolen. Just disputed the charge with citi. The question is, I use the card only in local stores and paypal transaction. How can the number be stolen?
 
1. Your computer. A keylogger or virus.
2. A skimmer on a card reader you used.
3. An employee copied your number down. Probably most likely at a restaurant, where your card leaves your sight.
