Anyone else receive this? It says required by law and it asks some detailed questions such as when we leave for work every morning and how many days we were sick last year.
 
I'm sure it is a scam, it's nobodys business when you leave for work unless they intend to use that information to do you harm.

Just beyond that it says if you willingly don't do it they can raise the fine to $5000. 🙁
