If it has never been changed and you have over 100k it could be a little dirty. My mazda b4000 had a dirty Mass airflow sensor and I was able to buy it cheap as the owner and his mechanic could not find the problem.

The ONE that it has was changed about 2 months ago

Check your pre-cat. It's right below the first O2 sensor. They're notorious for clogging at around 100K miles.

Usually, but I have heard of them going bad but not flashing the CEL.
