Here goes:

1)D, I haven't really used any extras yet
2)B,C, and D. I do ALOT of research before I make a major buy. I'll look at reviews with a grain of salt, weigh in brand history, and ask friends including other posters' experiences.
3)A, with the extra condition that it looks good in 3D, I'll sacrifice a little speed for better image quality any day.
4)I have owned mostly 3dfx(Diamond Monster & MonsterII) and ATI.
5)A, can't beat prices online
6)ME!! No one, and I mean no one touches my system;)

Image Quality=4
Speed=4
TV Out=2
Digital Support=1
Video In/Out=2
DVD Playback=4
Ease of Installation=3 (how hard can it be?;)
Driver Support=4 (I know, I know, I'm using ATI, they're improving and I've not had major probs yet:eek:)
Tech Support=2 Anandtech and I ARE tech support:D

Hope this helps!

2d is most important, i wont buy a graphics card if it doesnt have good sharp 2d but was as fast as a gf2 ultra (which ironically doesnt have good 2d, as say a voodoo3 or a rage128). Compatibility thats kinda vague, if i can get it to work , then that is good, though if it gets crippled that sucks. FPS isn't all that important, i guess it should be fast enough to run my games at around 45 fps, FSAA is important now that i've used it. So 2x fsaa @ 800x600x16bit and 45fps and i'm happy, i'm a very casual gamer.

4) What brand do you usually buy for a graphics card? 3dfx, matrox, ATI, ASUS (ASUS for geforces, i can get them real cheap and they are generally better 2d, i probably look at them in that order too, also depends on price and somewhat on 3d speed like with the matrox)

5) Where do you go to upgrade your graphics card?
a) Internet
d) Used
e) Other (please specify) my dad's computer stripdown friend

6) Who installs your graphics cards?
a) Yourself

d) Compatibility (OpenGL, DirectX features) ? not somuch an issue these days since every card coming out supports directX and this is the major API, but always ok to have support for others

4) What brand do you usually buy for a graphics card?
Right now I have a matrox G400 because of the DH, but the next one I?m getting, (next generation) will be something of Nvidia, ATI or maybe 3dfx. My last card was a Creative TNT2 Ultra, it was pretty sweet.

5) Where do you go to upgrade your graphics card?
a) Internet (maybe, if the store lists well on resellerratings.com
b) Brick and Mortar Stores (Circuit City, Best Buy etc.)

6) Who installs your graphics cards?

5 ? a card stands or falls with decent drivers
Technical Support
