Airline Miles Check Card

I just noticed that B of A is offering new check cards that give you miles back for your purchases. It is the same as my regular check card, just with the added benefit of earning miles and a $30 annual fee...worth it?

Also, how do the miles work. I would earn 1 mile for every $2 I spend, do I just calculate the number of miles associated with my trip and then get a free ticket if I have enough? Like if I were to spend $12000 and get 6000 miles. Would I be able to travel anywhere the airline flies within 3000 miles one way?

The choices are US Airways, Alaska Airlines, and America West...which one would be best?

I have the Alaska Airlines card and I've earned about 4500 miles thus far. I can switch those and combine them with my American Airlines miles. 25,000 miles is a free domestic ticket.
