Advanced Word user please help

I want to know how to create the fill in box like this doc. Further more, if I can add math function in it.

For inserting math equation:

Insert -> Objects -> Micrsoft Equation

or you can also use mathtype if you have it..

• create forms that users complete in Word

Do a search. Follow the directions.
