Usually will happen when working two jobs with decent pay. Neither jobs knows what the other withheld, the sum of the two can put you over the limit.

There is currently a max contribution, at least until SS is "fixed" to end the "crisis." I've no idea what you need to do since it only happened to me with consulting income (self employed, just didn't pay the extra).
