Don't know if this is your style but:

Black slacks with a dark colored dress shirt (Not the cheap ones with a pocket) and no tie.

pants - yes
sportsjacket (blazer) - i say yes because it looks classier than just a shirt and if it gets cold you can offer it to her for extra points

EDIT: shoes - yes, get something trendy but subtle

No tie. Just a button down shirt with the top 2 buttons undone, your hairy chest showing and a gold chain. Don't forget the grease.
