Yep, Photoshop. The effect is closer to what you can get with a tilt-shift lens than a narrow DOF anyway. Thanks for looking.

I believe the aperture was f/8. I processed it a fair bit during the B/W conversion. A good place to start:

One thing I didn't realize until just now: since the lens blur effect destroys so much contrast, it can be good to zap the contrast just after the blur, before undoing the quick mask. Another thing that seems nice to do is, instead of reflecting the gradient, do two separately. This allows a little bit wider "field" without really weakening the effect.
