A box of slicks isn't a bad idea :)
Actually, the floor mats and/or vanity plates arn't a bad idea. Maybe a key chain or sum'n.

How bout one of those crown air fresheners? Naked lady mud flaps?

Actually, the thing I like the most is when car owners have a model of their car sitting on the dash. It's even better when their car is a rusted out bondoed Charger. :D

How about an "Emergency Kit"...

This might include a tire inflator, insulated blanket, some of those heat packs (where you open the package and they get warm), flares, "send help" signs, jumper cables, etc.

Not a bad idea at all.

I might hit autozone after work and find something.

AAA membership

In case she ever needs her locks popped, runs out of gas or has a flat tire, she won't be stranded. This is assuming that her car insurance doesn't already cover this (my AmFam.com includes this coverage in my policy).

I like the AAA idea. Maybe you can talk to other people who've owned the car for a while and ask them what the thing they need most is...

Just a suggestion but I don't think I would go for any car accessories; I'd get something completely unrelated.

Create a set of free car wash "coupons"

and you will do the car wash for her

you can also create some "funny" small prints(coupon not valid on PMS day) on the coupon.

An emergency kit? Oh boy, there's a gift that'll make her want to jump your bones right on the spot. I bet she tears your clothes off right there, as soon as she opens her present. oooooooh...jumper cables...they make me soooo hot....:p

Seriously, it's a reasonable idea to want to get her something car-related, but I'd hope you could be more creative than an emergency kit, as well intentioned as the gift might be. :)

Personally, I'd go with something completely unrelated.

HAHahahaha, maybe this and some other unrelated gift. I always give her "gag gifts". Last year for christmas, I gave her a picture frame and in the center, I put a picture of her sleeping in the car during a road trip, head tilted back, mouth open. She was mad but she laughed. Time to go to the mall.... I hate that damn place.

Detailing grab bag

Zaino - Exterior
Invisiglass - Glass
Vinylex - Vinyl, plastic, rubber inside

Should be about $50
