Good only if "you were asked to provide your address or telephone number in conjunction with a return of merchandise for credit card credit of $10.00 or more at any California Walgreens store between June 1, 2004 and December 31, 2005; and (2) either (a) attaching a Walgreens receipt for that return or, (b) if you do not have your receipt, providing alternative information about your return to verify that you are a Class Member."
