Is running my CPU fan at 4600rpm all the time going to wear it out especially quickly? Or is it safe? I can only change the rpm via the manual dial and I'd rather just have constant maximum cooling than to be switching all the time.

Is 4600 rpm the rated RPM @ 12V for the fan? If so, then you shouldn't worry. So long as you aren't overvolting the fan, and making it run faster than it is designed for. While the speeds are putting wear on the bearing, it is doing so at a r ate of which the fan was designed for. So no, it won't put any additional wear on it, nor make it wear out exceptionally quickly.
