30 day notice for leaving an apartment, what do I say?

I'm moving soon, so I need to hand in a 30 day notice. I've never written anything like this, what should I say? "Hi, I'm moving out on so-and-so date. Thanks"? Or is more detail required? Thanks

After a very pleasurable 9 months of living at 366, I am writing to notify you that we will be moving out in thirty days, Monday, June 17th to be exact. We should have all of our items out by 3:00 in the afternoon, so if you would like to do the walkthrough any time after that, it would be welcomed. If you would like to do the inspection earlier in the day, we will be able to accommodate that, just let us know.

Also might want to put a deadline on when they can let you know if they want to inspect earlier in the day.
 
How about:

Dear landlord,

Due to the increasing severity and overall contagion level of my yet unnamed syndrome/disease, I will need to move close to the university medical center to be closer to researchers.

I will be moving in 30 days unless you see it necessary to hasten my relocation.

My advice: make two copies and have you and your landlord sign and date both of them so you can keep one. That way they can't pull any "we didn't receive it on time" BS later. I learned this the hard way. 🙁
