3.5" drive bay ports

I am trying to find a 3.5" drive bay port adder that has exactly 1 headphone jack, 1 USB 2.0 port, and 1 firewire 400 port. The closest thing I have found is this

Also it must be black to match the rest of my case. Anyone have any suggestions?

I would go find a specific link, but the site is b locked for some reason at work. But if you go to Performance PCs, they have a section for your selection.

I might be able to answer my own question here. After much more googling, I found this site.

Unfortunately, their custom bays are a lot more expensive than buying a pre-designed one and having a few dead ports. Also, they don't seem to have 3.5" bays. Anyone used these before?
