2001 Civic rough at startup

Backstory:

The civic has been in Germany with my family for 2 years b/c that was his duty assignment (military). 1 month ago it was shipped back to the states and I picked it up in Baltimore and drove it back home (blacksburg, va).

Problem:

On startup after letting it sit for a day the RPMs will drop from 1500 to less than 500, the engine will run very rough, and then it will return to normal. 20 seconds later it will repeat the same thing. This goes on for about 5-8 cycles and then the car runs perfectly fine (40 mpg, smooth acceleration and shifting).

Shutting it down and then restarting it after an hour or so does have the same effect, it works perfectly.

Attempted fix:

On startup I've tried revving the engine to about 2500-3000 RPM but that doesn't seem to have any effect.

Question:

What is going on?
Can it be fixed by me (very noobish w/ cars)?
How much should it cost to fix at a shop?

This is just sitting, no steering input and with the AC turned off? The periodic frequency sounds like the AC compressor kicking on and the idle up valve not working.

If it does this any time all the time warm or cold and only at idle, and does not affect acceleration or power ever, it sounds like a problem with the idle air valve or a small vacuum leak causing the engine to idle hunt.

Should be reasonably cheap to diagnose, not even an hour's worth of shop labor to check the basics.
