it's quite possible that he just played back a video of that on the computer while do it slow, sped it up and matched it with his keyboard while recording.
 
its real, stepmania is so easy with fingers. you just sit there, i've gotten B on Quasar before, and you should see me with real stepmania, i can hardly do the easiest and get a decent rating. My friend can do that insane korean one with 6 keys and its exponentially faster than stepmania.
