"On the night of April 13th, 1970, when the oxygen tank in Apollo 13’s command module exploded,"

Service Module; everyone knows this.

I knew most of those. But a few I didn't. It was an interesting read. It's nice to know somethings don't change much.

Foundations of
Mission Operations

1. To instill within ourselves these essential qualities of leadership in pursuit of technical and professional excellence...

Discipline...Being able to follow as well as to lead, knowing that we must master ourselves before we can master our task.

Competence...There being no substitute for total preparation and complete dedication, for space will not tolerate the careless or indifferent.

Confidence...Believing in ourselves as well as others, knowing that we must master fear and hesitation before we can succeed.

Responsibility...Realizing that it cannot be shifted to others, for it belongs to each of us; we must answer for what we do, or fail to do.

Toughness...Taking a stand when we must; to try again, and again, even if it means following a more difficult path.

Teamwork...Respecting and utilizing the abilities of others, realizing that we work toward a common goal, for success depends upon the efforts of all.

Vigilance... Always attentive to the dangers of spaceflight; Never accepting success as a substitute for rigor in everything we do.

2. To always be aware that suddenly and unexpectedly we may find ourselves in a role where our performance has ultimate consequences.

3. To recognize that the greatest error is not to have tried and failed, but that in the trying we do not give it our best effort.

Jesus hates NASA cuz they do science. If there's anything that Jesus hates, it's science.

I'm beginning to wonder if the OP even bothered to read the article he linked. And as someone who was around at the time of Apollo 13, I already knew most of that list. So I guess the OP is wrong about his guarantee, too.

Yea, I did read it, I just dun goofed. Sorry everyone. I was shocked to read that center engine was about to pound it's way into the fuel tanks, I had no idea it was that bad.

OP, I'm not sure if you're aware, but there's an Academy Award winning movie on Apollo 13 that focused on several of these 13.
Yea, I've seen Apollo 13, (more than once too) but it never got into the detail this article did. Like the bad O2 sensor that had them stirring the tank more than usual. If not for that situation the tank would have been stirred when the LEM was on the surface, (it exploded on the 5th stir) and all would have died. No LEM "lifeboat" and the CSM in God knows what orbital attitude after the explosion. Remember when they docked it was the CSM that did the maneuvering to enable docking.

Don't forget, among those 13 reasons, there was a LOT of speculation. If that 5th stir didn't occur until they were in lunar orbit, the conditions inside that oxygen tank would have been different - perhaps it wouldn't have exploded at all.

We just don't have a good time with number "13" missions.

I was lead for my system during ISS assembly mission 13A and that flight had some significant problems. Duct tape was a solution to one of those problems too.
