I do have a 120mm fan on the bottom front of the case, yes. I put a side window in my case so I don't have the side intake fan anymore, on an unrelated note my temps seem 2c-5c better without it, so I don't know if that would impact airflow as far as the PSU is concerned.
I have an Antec 300 case and there's no vents in the bottom so I have the PSU fan pointing up, this causes the attached wires on the PSU to be on the wrong side cluttering things up.

Might there be something smaller? I'm not sure that'll fit on my motherboard, an MSI-K9N6PGM2-V2, it looks like a line of capacitors would get in the way.
I have a stock AMD heatsink/fan currently and it's the loudest fan in my computer, and very distracting while watching movies or gaming. For a budget of $30-$40 what would be the quietest thing I can find for my CPU?
I was able to get about 4 hours worth of gameplay in today with only one error which happened when I tried launching a second instance of the game, it looks like manually setting the voltage to 1.325v worked. Thanks for the help, it's appreciated.

I removed my heatsink and took a look at my CPU, it is this one:

which is a 95w CPU. I ran memtest with the voltage manually set to 1.325v and it went through without an error.
I ran memtest with everything set to auto before I started any of the above listed steps and it made it through the tests without errors. I'll do it again with the voltage set manually to 1.325v (as high as my BIOS goes) in a moment.
My motherboard supports up to 95w CPUs including the CPU...
