Issue is officially resolved! Just scored 1250 pts on Unigine Heaven 4.0 completely maxed out, on factory clock speeds on both video card and cpu. I figured it out never gonna believe what it was!

Just ran unigine heaven 4.0 again down to 863 pts. I believe my video card is failing. The score seems like it keeps going down and down. And I'm not changing any settings.

What does throttling mean? Um well with stock clocks it never exceeded 77 Celsius then I over clocked it which is what gave me the score in the picture buy quite a bit like I think I pushed it to 1300 mhz gpu clock and it still never broke over 77 Celsius.
