This is not something that can be done. Implementing moderation measures like that would have a blanket effect, requiring all posts from all users to be approved before being published.

This is intentional, due in part to the fact that the 'Home' icon, which also leads back to the main editorial page, is very tiny and easy to miss.

I have a hard time believing this since actual broken avatars lead to broken links whereas yours leads to your image—a blown up version of the broken avatar icon, which displays just fine.
