I wouldn't get an Insignia HDTV...there are plenty of LG, Sharp, Toshiba, etc. sets for around $400 if you're patient and search around. Take a look at the LG 42CS560 on Amazon, it's about $450 and has great picture quality/reliability.
Refurbs, for the most part, are quite reliable..but there are always a few exceptions. I would just exchange this one; I doubt such an error will occur again.

The Audio-Technica ATH-M50 might be a reasonable choice..or the Sennheiser HD280 Pro. Take a look at the features and pricing.

I'm not certain that the iPhone 5 will have enough new features over those of the 4s to warrant another purchase, but usability, durability, and comfort will definitely be factors in that decision.
I second this. Vizio is a fine brand with plenty of quality sets, as long as you take the time to find the model which satisfies your interests. Take a look at reviews and try to find an above-average deal for the TV that you want; it doesn't need to be 'top-tier' to have reasonable quality...
