Try installing HLS with all the options checked (including CS source code)during installation.
When I unchecked CS sourcecode option, it also gave me an error in CD4.
It took almost an hour to find what's wrong on internet, but basically Valve put so much effor into...

I also received red a PCB initially, and it gave me a lot of errors even at 333Mhz. I RMAed it, and NEWEGG sent me a green PCB. Now the problem was gone. Try to ask NEWEGG to give you green PCB when you RMA it.

Mmmm. after hearing good stories of green PCB, I am wondering PCB actually makes difference.
Mine was running at 166Mhz (2.5-6-3-3), and still giving errors in memtest. Voltage upto 2.6 did not solve the problem either.
Good thing is that RMA process was very easy. Still.... i need to pay for shipping...
